Consensus Document on Clopidogrel and PPIs

Since there is still a little bit of uncertainty on the use of clopidogrel together with protonic pump inhibitors (PPIs), a consensus document has been published by the American College of Cardiology, by the American College of Gastroenterology and by the American Heart Association. We know that clopidogrel is competitively inhibited by PPIs. Its action in reducing platelet aggregation is then antagonized by PPIs, even if clinically we do not know well which is the extent of this inhibition.

Selective Renal Sympathectomy in Resistant Hypertension

According to both animal studies and human studies, it results that renal sympathetic denervation, performed through catheter, is capable to decrease blood pressure resistant to usual treatments. In the first randomized trial on this technique (sponsored by the producer of the used catheter), 106 patients with resistant hypertension (systolic pressure ≥ 160 mmHg in non-diabetic subjects and systolic pressure ≥ 150 mmHg in diabetics, all under treatment with at least three anti-hypertension drugs) have been randomized to undergo sympathetic catheterism.

We know, from previous trials, that cardiovascular risk decreases in a linear way with the reduction of LDL cholesterol, obtained through statin administration (for example, CV risk decreases of 20% for each reduction of 40 mg/dl in LDL cholesterol).

New ESC Cardiovascular Disease Guidelines Emphasize Primary Prevention

The emphasis is on the lifelong prevention of cardiovascular disease, say the experts, likening the new European Society of Cardiology (ESC) cardiovascular disease prevention guidelines to the owner's manual of a new car. While the car is all bright and shiny and new, owners still need to read the instructions because only a maintained engine hums along at full capacity. This approach to cardiovascular disease prevention, which ideally starts with young parents and continues with children throughout their years in school, is intended to emphasize heart-healthy lifestyles and behaviors. For adults, risk-adjusted prevention can be coordinated through nurse-based activities and by the preventive efforts of general practitioners and cardiologists.

Testosterone supplements helped heart failure patients breathe better and exercise more, according to research in Circulation Heart Failure, an American Heart Association journal. Researchers analyzed four randomized clinical trials of patients with moderate to severe chronic heart failure. Patients were given commercial testosterone supplements by injection, patch or gel. Based on the analysis of these studies, those who received supplemental testosterone scored 50 percent better in a six-minute walking test than those receiving placebo.
