Clinical Imaging: Pneumoperitoneum

Plain-film radiographic images of a chest can reveal evidence of pneumoperitoneum, with gas extending from the infradiaphragmatic region to the inferior margin of the liver, outlining the gallbladder. Moreover, gas outlined the spleen on the lateral view can be highly suggestive of bowel perforation.

Methacholine challenge - a good test to diagnose asthma?

In a manuscript in The Journal of Allergy and Clinical Immunology (JACI), Sumino et al and the American Lung Association Asthma Clinical Research Centers examined the utility of the MCT on asthmatics who are on controller medications. The researchers evaluated the sensitivity (percentage of asthma patients who are correctly identified as having asthma) and specificity (percentage of healthy people who are correctly identified as not having asthma) of MCT in 126 patients with asthma diagnosed by a physician, and 93 normal healthy individuals.

Reducing Breath Size and Pressure From ICU Ventilator Increases Long-Term Survival in People with Acute Lung Injury

Carefully adjusting mechanical ventilator settings in the intensive care unit to pump smaller breaths into very sick lungs can reduce the chances of dying by as much as 8 percent, according to a study by critical care experts at Johns Hopkins. Study participants were evaluated for two years after their acute lung injury.

Impulse oscillometry (IOS) identifies uncontrolled asthma in children

Assessing asthma control in children is particularly challenging: impulse oscillometry (IOS) being a new non-invasive technique has been increasingly used for the diagnosis of asthma. IOS is effort-independent and able to separately quantify the obstructions in proximal and distal airways, but has not previously been utilized to assess asthma control in children. In a recent article in The Journal of Allergy and Clinical Immunology (JACI), Shi et al. measured IOS in 101 children (6-17 yrs) with mild-to-moderate asthma.
200th Anniversary: a Patient with Asthma Seeks Medical Advice

People have suffered from asthma for millennia. Although the clinical presentation of asthma has probably changed little, there are many more people who now bear its consequences than there were 200 years ago. As a result of an intense interest in the condition, understanding of its pathobiology, how to diagnose it, and — most important — how to treat it has evolved dramatically over the past two centuries. To illustrate this change, authors provide three fictional reports of consultations performed for essentially the same patient, who has what we in 2012 would refer to as asthma.

Respiratory syncytial virus (RSV) and human rhinovirus (HRV) are the most common viruses associated with infant acute respiratory infections (ARI). Both RSV and HRV lower respiratory tract infections (LRTIs) during infancy and early childhood are associated with an increased risk of asthma later in childhood.

Standardized Outcome Measures Proposed for Asthma Clinical Research

A consortium of federal agencies and nongovernmental organizations has published a report proposing a set of common measures and data-collection methods for use in asthma clinical research. The report identifies standard definitions and methodologies for measuring seven groups of outcomes that are important in asthma clinical studies: symptoms, exacerbations (asthma attacks), biomarkers (biological substances that indicate disease state), lung function, quality of life, asthma control questionnaire results, and health care utilization and costs.

A 50-year-old woman presented with fatigue and shortness of breath. One week before presentation, she experienced the gradual onset of dyspnea after moderate exertion, profound malaise, and a non-productive cough.
According to observational trials, among the various negative effects of vitamin D deficiency, also a greater easiness towards respiratory infections has been described. Since during the long winter in Arctic countries vitamin D deficiency is a usual occurrence, some researchers in Finland have arranged a strict, blind trial, administrating vitamin D to healthy adults and controlling the working days lost because of respiratory infections.

Silent Pulmonary Embolism in DVT

According to post-mortem results, it has been seen that the diagnosis of pulmonary embolism, even in cases of important extension, is often not performed, so researchers have wondered whether pulmonary embolism screening should be performed in patients with deep venous thrombosis (DVT), even in the absence of respiratory symptoms. A systematic review has been performed on 28 prospective trials to verify the prevalence of pulmonary embolism in patients with DVP.
