    Ranibizumab resulted effective in improving best corrected visual acuity (BCVA) and is well tolerated in DME. Future clinical trials are required to confirm its long-term efficacy and safety.

  • The Role of Blood Pressure Variability in the Development of Nephropathy in Type 1 Diabetes

    Visit-to-visit variability in blood pressure consistently independently added to mean blood pressure in predicting the risk of nephropathy, but not retinopathy, in the DCCT. This observation could have implications for the management and treatment of blood pressure in patients with type 1 diabetes.

  • The Effect of Iron and Erythropoietin Treatment on the A1C of Patients With Diabetes and Chronic Kidney Disease

    Both iron and ESA cause a significant fall in A1C values without a change to glycemic control in patients with diabetes and CKD. At the present time, regular capillary glucose measurements and the concurrent use of CGM remain the best alternative measurements of glycemic control in this patient group.

  • Obesity, Diabetes, and Gut Microbiota: The hygiene hypothesis expanded?

    The connection between gut microbiota and energy homeostasis and inflammation and its role in the pathogenesis of obesity-related disorders are increasingly recognized. Animals models of obesity connect an altered microbiota composition to the development of obesity, insulin resistance, and diabetes in the host through several mechanisms: increased energy harvest from the diet, altered fatty acid metabolism and composition in adipose tissue and liver, modulation of gut peptide YY and glucagon-like peptide (GLP)-1 secretion, activation of the lipopolysaccharide toll-like receptor-4 axis, and modulation of intestinal barrier integrity by GLP-2.

    Preceding the joint meeting of the 19th annual Diabetic Neuropathy Study Group of the European Association for the Study of Diabetes (NEURODIAB) and the 8th International Symposium on Diabetic Neuropathy in Toronto, Canada, 13–18 October 2009, expert panels were convened to provide updates on classification, definitions, diagnostic criteria, and treatments of diabetic peripheral neuropathies (DPNs), autonomic neuropathy, painful DPNs, and structural alterations in DPNs.

  • Neuropathy, Retinopathy, and Glucose-Lowering Treatments

    Many studies presented at the American Diabetes Association (ADA) Scientific Sessions addressed aspects of neurologic disorders in diabetes. Braza et al. (abstract 569) found that 19% of 76 type 2 diabetic persons receiving metformin for >1 year had vitamin B12 levels <250 ng/ml, and 23% had levels 250–400 ng/ml. (Abstract numbers refer to the Abstracts of the 69th Scientific Sessions of the American Diabetes Association, Diabetes, Vol. 58, Supplement 1, 2009.) Peripheral neuropathy was present in 77 and 23% of these groups, respectively, and was found in 7% of those with normal B12 levels. Deficiency correlated neither with duration of metformin use nor with erythrocyte macrocytosis.
