  • Updated guidelines on non-invasive heart op

The procedure, called transcatheter aortic valve implantation (TAVI) involves replacing the narrowed aortic valve with an artificial one, which is inserted into the patient's heart through a catheter. This is an alternative to open-heart surgery (surgical aortic valve replacement, SAVR) in which the aortic valve is replaced by opening up the chest.

  • Endarterectomy in Asymptomatic Carotid Stenosis

In patients presenting a moderate-severe but asymptomatic carotid stenosis, endarterectomy surgery decreases the risk of stroke in the long term, but it drags behind itself the risk of intraoperative stroke and death. In a previous trial, started in 1993, 3120 patients in these conditions had been randomized to perform an immediate endarterectomy or only to undergo close controls (with the indication to choose surgery for any reason and in any moment). In a report in 2004, it had been seen that immediate endarterectomy presented a perioperative risk of stroke or death equal to 3%, while the risk of stroke at 5 years was significantly lower than in the group of watchful waiting (3.8% vs. 11.0%).

  • Trends in isolated coronary artery bypass grafting: An analysis of the Society of Thoracic Surgeons adult cardiac surgery database

Coronary artery bypass grafting (CABG) is the operation most commonly performed by cardiac surgeons. There are few contemporary data examining evolving patient characteristics and surgical outcomes of isolated CABG. Trends in patient characteristics and outcomes after CABG over the past decade have been characterized using the Society of Thoracic Surgeons adult cardiac surgery database.

  • Dobutamine administration in patients after cardiac surgery: beneficial or harmful?

A discussion about a retrospective analysis showing that dobutamine use in patients undergoing cardiac surgery was independently associated with a 2.3-fold increase in mortality and a 2.7-fold increase in risk of renal dysfunction.

  • Two Approaches Help Identify Cardiac Surgery Patients at Risk for Postoperative Kidney Failure

After coronary artery bypass grafting (CABG), between 5 and 30 percent of patients may experience acute kidney injury, defined as a rapid loss of kidney function. Once an acute kidney injury develops, there is an increased risk for poor outcomes, such as a threefold increase in 30-day mortality rates. Two criteria methods are used to define the type of acute kidney injury and its consequences. A recent study found that both sets of criteria were accurate as early predictors of mortality from acute kidney injury among CABG patients after surgery.
