  • Allopurinol in Stable Angina

    From experimental evidence, we know that allopurinol reduces oxygen use by myocardium, so researchers have wondered whether this drug is capable to obtain some benefits in patients with heart ischemia. Some English researchers have performed a crossover trial randomizing 60 subjects with strain-induced stable chronic angina, shown with angiography, with positive ergometric test, to add also 600 mg/die of allopurinol or placebo, for 6 weeks, to their usual therapy.

  • Self-Control of Hypertension

    It is an already assessed element the fact that half patients under treatment for hypertension do not present acceptable pressure values, so, researchers have wondered: if self-control for asthma or for coagulation is a consolidated and effective practice, can it become so in the case of hypertension, too? Some English researchers have randomized 480 adults with non-controlled hypertension (>140/90 in spite of the treatment with one or two drugs) to the usual therapies or to self-control: patients in the self-control group have measured pressure with automatic sphygmomanometers twice in the morning, for a week in a month.

  • Statins in Primary Prevention

    Given their preventive efficacy in patients with heart diseases (a decrease in overall mortality), can we consider the use of statins in primary prevention (that is to say in patients with CV risk but not affected by heart diseases yet)? Some European researchers have extracted from 11 randomized trials the results referred only to high-CV-risk patients but with no CV diseases.

  • Reduction of Homocysteine (SEARCH Trial)

    It is well known that plasmatic levels of homocysteine are associated with an increase in cardiovascular events, but the decrease of this amino-acid, obtained through folic acid, has not produced the expected results, at least according to some randomized trials recently performed. Since these trials have been criticized for a too small sample or a too short duration, some English researchers have randomized about 12,000 high-risk patients (survived to myocardial infarction, averagely 64 years of age) to receive 2 mg of folic acid and 1 mg of vitamin B12 or placebo daily.

  • Metabolic Syndrome and Infarction Risk

    The so-called metabolic syndrome (whose presence has been contested by many clinicians) is presented by metabolic disorders such as dyslipidemia, abdominal obesity, hyperglycemia and hypertension. One of the aspects under discussion is the quantification of cardiovascular risk: is it higher than the addition of the single elements or is it given only by the presence of multiple situations at risk? With a wide case-control trial on myocardial infarction, over 16,000 subjects (6905 with infarction) matching the criteria of the definition of metabolic syndrome have been examined.

  • Walnuts, Almonds and Lipids

    In various epidemiological trials, the association between nuts consumption and coronary risk decrease has been assessed. With an overall analysis on 25 trials performed in 7 different countries and involving 583 participants, the relation between walnuts and/or almonds and plasmatic lipids has been assessed. The analysis included 16 crossover RCT. The only intervention on diet in all the trials was the adding of nuts to usual nutrition. No participants received lipid-lowering treatments and they have been excluded from the trials where there was a huge weight loss.
