Immunosuppressive induction therapy—routine in organ transplants—reduces the risk of organ rejection but is associated with adverse effects. Infusion of bone marrow–derived mesenchymal stem cells, which have immunoregulatory effects, may offer an alternative immunosuppressive approach. In a randomized trial of 159 patients undergoing living-related kidney transplants, Tan and colleagues found that compared with conventional anti–interleukin 2 receptor antibody–based therapy, a regimen that involved infusion of autologous mesenchymal stem cells was associated with a lower incidence of acute rejection and better renal function at 1 year.

An 85-year-old man is admitted for evaluation of multiple papules, plaques, and subcutaneous nodules on his face, trunk, and extremities. The patient has itching. There is no history of trauma. He has an 8-year history of diabetes and Sjögren syndrome with eye and mouth involvement and has been treated with varied doses of oral methylprednisolone for 5 years.

Variants in the CYP2C19 gene influence the pharmacologic and clinical response to the standard 75-mg daily maintenance dose of the antiplatelet drug clopidogrel. A study tested whether higher doses (up to 300 mg daily) improve the response to clopidogrel in the setting of loss-of-function CYP2C19 genotypes.

Games are now a dominant form of media, even larger than the motion picture industry, and are enjoyed across gender, age, and cultural boundaries. Games targeting healthy behaviors are also proliferating. Such games deserve serious attention, because clinicians and policy makers will be confronted with decisions regarding their use. There is great promise in channeling these hours of engagement to address some of the most difficult and persistent challenges involving diet, exercise, and adherence to therapy. Sufficiently engaging games might enhance the effectiveness of health messaging, allowing individuals to practice useful thought patterns and behaviors and encouraging them to explore and learn from failure in safe virtual environments.

In a a prospective community-based cohort study started in 1948, familial AF was associated with an increased risk of AF that was not attenuated by adjustment for AF risk factors including genetic variants. Assessment of premature familial AF was associated with a very slight increase in predictive accuracy compared with traditional risk factors.

Pain, a common complaint for elderly patients, can be challenging for physicians to treat safely and effectively. To better understand these challenges and to promote safer use, the US Food and Drug Administration (FDA) has recently begun examining pain medication use in elderly patients as part of its Safe Use Initiative.
