  • Consensus Document on Clopidogrel and PPIs

    Since there is still a little bit of uncertainty on the use of clopidogrel together with protonic pump inhibitors (PPIs), a consensus document has been published by the American College of Cardiology, by the American College of Gastroenterology and by the American Heart Association. We know that clopidogrel is competitively inhibited by PPIs. Its action in reducing platelet aggregation is then antagonized by PPIs, even if clinically we do not know well which is the extent of this inhibition.

  • Selective Renal Sympathectomy in Resistant Hypertension

    According to both animal studies and human studies, it results that renal sympathetic denervation, performed through catheter, is capable to decrease blood pressure resistant to usual treatments. In the first randomized trial on this technique (sponsored by the producer of the used catheter), 106 patients with resistant hypertension (systolic pressure ≥ 160 mmHg in non-diabetic subjects and systolic pressure ≥ 150 mmHg in diabetics, all under treatment with at least three anti-hypertension drugs) have been randomized to undergo sympathetic catheterism.

  • Statins and Cardiovascular Risk

  • Dabigatran in the Prevention of Stroke and TIA

    In the RE-LY study, on patients affected by atrial fibrillation and with a stroke risk, the administration of dabigatran (an oral inhibitor of thrombin) has reduced the risk of stroke and has presented a bleeding risk superimposable to the ones of warfarin.

  • Venlafaxine in Generalized Anxiety Disorder

    The generalized anxiety disorder (GAD) is a chronic disease needing long-term treatments. In order to verify the obtainable benefit with a 6-month and a 12-month therapy through long-term-release venlafaxine, 268 subjects with a diagnosis of GAD have been treated through an open-label study: after 6 months of venlafaxine treatment, improved patients have been randomized to receive 6 further months of venlafaxine or placebo.

  • Romiplostim for Immune Thrombocytopenia

    In the treatment of thrombocytopenic purpura, the used drugs are the ones capable to stimulate platelet production, even if these products, even obtaining an increase in the number of platelets, are not often capable to reach clinically significant results, antagonized as they are by the increased platelet destruction, typical of purpura.

  • Dabigatran in Atrial Fibrillation

    Dabigatran is an inhibitor of thrombin, authorized for its use in the prevention of stroke in patients with atrial fibrillation, showing the efficacy and the safety of warfarin but with some advantages compared to this one.   • Bisphosphonates and Risk of Hip Fractures

    For the treatment of postmenopausal and of cortisone-related osteoporosis, the primary role is kept by bisphosphonates, even if some studies have suggested a possible link between the continued use of these molecules and the onset of low-energy hip fractures (subtrochanteric or diaphisary ones), a consequence of an interference with bone reabsorption.

  • Survival in Patients with Advanced Dementia

    One of the questions with no answer is the one relative to survival in patients with an advanced stage of dementia, and the answer to this question often influences the decisions of possible hospitalizations in palliative care structures. With a prospective study, the prevision of death among 606 patients hospitalized in a protected structure in Massachusetts have been examined, according to two parameters: the ADEPT (Advanced Dementia Prognostic Tool) score, an evaluation scale including 12 factors, among which age, presence of trophic ulcers, incontinence, recent weight loss, where the scale ranges from 1 to 32.5 points and where the highest score indicates a lower survival.
