The burden of cancer is growing rapidly, and the disease is becoming a major challenge for all developed countries. What are the drivers and solutions to the so-called cancer-cost curve in developed countries? The Lancet Oncology Commission on delivering affordable cancer care in high-income countries collects
expert opinion from health-care professionals, policy makers, and cancer survivors has been gathered to address the barriers and solutions to delivering affordable cancer-care in high-income countries.

Toll-like receptor 9 (TLR9) is a cellular DNA-receptor whose activation with cognate ligands triggers an immune reaction, with increased production of inflammatory cytokines. The aim of this study was to examine the expression of TLR9 in renal cell carcinoma (RCC), which is generally renowned of its immunogenic nature. TLR9 expression resulted as an independent prognostic marker of RCC and the absence of TLR9 expression is related to poorer prognosis in RCC.

After undergoing breast lumpectomy and biopsy of the sentinel lymph nodes, a 42-year-old woman with triple-negative invasive ductal carcinoma began adjuvant chemotherapy with doxorubicin, cyclophosphamide, and paclitaxel. After the second cycle of doxorubicin and cyclophosphamide, asymptomatic hyperpigmented macules and patches were noted on the patient's tongue.

In 2003, the CHARM (Candesartan in Heart failure Assessment of Reduction in Mortality and Morbidity) study has shown a slight but significant increase in cancer-related deaths in patients treated with candesartan, compared to placebo. Now, a meta-analysis on randomized trials on angiotensin-II-receptor antagonists has been performed as to the problem of cancer. In most trials, telmisartan has been used. In the 5 trials reporting the incidence of new cases of cancer, the overall incidence of tumours has been higher in the subjects treated with sartans than in controls (7.2% vs. 6.0%; RR 1.08; IC 95% 1.01-1.15).

Self-renewing breast cancer stem cells are key actors for perpetuating tumour existence and for treatment resistance and relapse. The molecular pathways required for their maintenance start to be elucidated. Among them is the transcription factor NF-kappa B, which is known to play critical roles in cell survival, inflammation and immunity. Recent studies indicated that mammary epithelial NF-kappa B regulates the self-renewal of breast cancer stem cells in a model of Her2-dependent tumorigenesis. We will describe here the NF-kappa B-activating pathways that are involved in this process and in which progenitor cells this transcription factor is actually activated.

Using the latest gene sequencing tools to examine so-called epigenetic influences on the DNA makeup of colon cancer, a Johns Hopkins team says its results suggest cancer treatment might eventually be more tolerable and successful if therapies could focus on helping cancer cells get back to normal in addition to strategies for killing them.
