    Patients with chronic renal failure under dialysis treatment present high risks of mortality for cardiovascular causes. In order to verify whether also general mortality is increased, a cohort of 120,000 subjects recently enrolled in a dialysis program (average age 63; 61% males) in nine European countries has been studied.

  • Mechanisms of Disease: Genetic Disorders of Renal Phosphate Transport

    This review recounts the molecular mechanisms that control serum phosphate levels and describes the clinical consequences of abnormalities of these mechanisms. Several proteins in the kidney participate in reabsorption of urinary phosphate; the review describes mutations in the genes that encode these proteins, and the syndromes they produce.

  • Chronic Kidney Disease in Elderly Individuals

    Chronic kidney disease (CKD) increases the risk of progression to end-stage renal disease (ESRD) and is associated with greater risk of death and of cardiovascular outcomes. Because the incidence of atherosclerotic cardiovascular disease is so much higher in patients with CKD than in the general population, the National Kidney Foundation (NKF) Task Force on Cardiovascular Disease concluded that patients with CKD should be considered to be in the highest risk category (ie, a coronary heart disease risk equivalent) for risk factor management, and the American Heart Association now recommends that all patients with cardiovascular disease should be screened for CKD.

  • Sartans in Renal Prevention

  • Renal cell cancer without a renal primary

    Renal cell carcinoma has been increasing in incidence over the past two decades. Men are affected more than women and metastatic disease at presentation occurs in up to one third of patients. Metastasis can occur to virtually any organ, and involvement of multiple organs is not uncommon. To date, no reports have been found of metastatic disease without a renal primary. We present a case of renal cell cancer initially presenting as a subcutaneous mass with subsequent pancreatic and parotid gland metastases in absence of a primary renal source.

  • Mild Hyponatremia and Prognosis

    As to hospitalized patients, the presence of hyponatremia is associated with a worse prognosis, but there aren’t many data on the association between hyponatremia and prognosis in the general population. Some researchers in Denmark have evaluated plasmatic levels of sodium in 671 subjects averagely 65, not affected by heart diseases, stroke, cancer or other important diseases.

  • Renal Function and Cardiovascular Risk in Women

    From previous trials, we know that the decreased function is associated with an increased cardiovascular risk and with a greater probability of death for any cause. But these trials are performed on high-cardiovascular-risk subjects, on subjects with present CV diseases and on subjects with impaired renal function. In order to evaluate nephrological importance in people with no such risks 28,000 women have been analyzed, enrolled in the Women’s Health Study, with no cardiovascular diseases and being at least 45.

  • Intensity of Continuous Renal-Replacement Therapy in Critically Ill Patients

    This multicenter, randomized trial compared the effect of continuous renal-replacement therapy at two different levels of intensity on 90-day mortality among critically ill adults with acute kidney injury. The higher-intensity therapy did not reduce mortality at 90 days.
