The Tdast is an excellent choice for reconstruction in the head and neck as an alternative to procedures requiring vein grafting and multiple free tissue transplants, or in which the 3-dimensional contour of the scapular tip aids in reconstruction. The complication rate should be assessed in the context of the risk factors of the patient population and the outcome with respect to stable employment, increasing body mass index, and maintenance of shoulder function.

Pediatric obstructive sleep apnea refractory to adenotonsillectomy that is due to retroglossal and base-of-tongue collapse remains difficult to treat. However, most patients in this analysis benefited from combined genioglossus advancement and radiofrequency ablation.

Pneumococcal vaccination has reduced but not eliminated childhood deafness secondary to pneumococcal disease. Auditory steady-state response testing and 3-dimensional steady-state free precession imaging are modalities that expedite candidacy evaluation of this population. Early bilateral simultaneous implantation increases the likelihood of binaural hearing and ensures implantation of the better ear in this population of children whose course is often complicated by formation of scar tissue and ossification within the cochlea.

It has been well established that a crisis exists in the biomedical research enterprise of clinical surgery departments: the extinction of the surgeon-scientist. A number of factors, including dwindling research funding, decreasing reimbursement from insurance providers, shrinking salaries for researchers, waning interest in academia among graduating residents, and financial pressures on departments for greater clinical productivity, have contributed to this phenomenon in recent years.

The presidential year is a time for reflection on our purpose as an organization and consideration of where we should place our efforts for the future. People are telic, they exist for a purpose and spend a great deal of time and energy finding that purpose and pursuing it. As a society of health care providers engaged in caring for people with the devastating effects of cancer of the mouth, throat, neck, and skull base, we share many personal purposes. This results in common goals that the American Head and Neck Society (AHNS) should attempt to reach together as an organization. The amount of time and energy each of us dedicates to AHNS will be directly related to how much commonality we feel in these shared purposes, how passionate we are to pursue them, and how well the Society functions in addressing them.

Rhinosinusitis was found to be the primary cause of TSS 21% of the time in this series. Rhinosinusitis should be considered the primary cause of TSS when another site of infection has not been identified. Once the link is made, prompt otolaryngology consultation and sinus lavage should be considered.

Choanal atresia and stenosis is associated with a wide range of disorders. Congenital anomalies, neurologic abnormalities, and developmental disabilities are commonly identified in affected individuals. Bilateral CA/S is more commonly seen in patients in whom specific diagnoses or other congenital anomalies are identified. Unilateral CA/S occurs more frequently in isolated cases. A comprehensive evaluation is recommended in individuals with CA/S to evaluate for other congenital anomalies, neurologic abnormalities, developmental delays, and evidence of a specific underlying disorder.

Recurrent respiratory papillomas have a characteristic ultrasonographic appearance that seems to correlate with endoscopic findings. It seems that this modality holds promise for identifying pedunculated papillomas. Although direct laryngoscopy and bronchoscopy are the criterion standard, airway ultrasound may have a role in the early diagnosis of, surveillance of, and operative planning for recurrent respiratory papillomatosis.
